Your personal information may include name, age, date of birth, sex, email address, phone number, address, payment information, auto fill data, emergency contact details, student identification details, employment details, and other sensitive information.
We may also collect your location data through our online platforms if you choose to enable this service. We may also collect personal information via Social Media, SMS mobile marketing or from a person’s use of the website via contact mailboxes or registration processes, or via mobile sites, applications, widgets and other mobile interactive features or QR Codes. Personal information is also collected when completing the membership agreement and a health questionnaire.
Our facility has surveillance cameras which may capture your image on film. This information may be used in respect of management and security at our facility and may be provided to law enforcement or government officials if requested.
USE OF PERSONAL INFORAMATION
We require personal information in order to meet our legal obligations, and provide products or services to you, to render services under a membership agreement, to record statistical data for marketing analysis of members, develop and adjust fitness regimes and nutritional guidance for your personal needs, provide you with information and offers or other benefits that may be available to you, to process or consider an application for employment, to promote or market our business, to obtain feedback of your opinion or suggestions for improvement of our business products and services, contact you to manage your membership and fulfil our obligations to you or your organization, and recover monies owing to use from time to time, and for other secondary purposes. If we are not provided with all the personal information we request, we may not be able to supply our products or services to you, and you or your organization may not be able to participate in future offers of services which we supply.
Inertia Fitness may engage external companies or service providers to assist us in providing our services including marketing, communications, debt recovery, product development, analysis, consulting, provision of services. Personal information will not be used for direct marketing purposes without the individual’s consent.

ALLIED HEALTH TERMS & CONDITIONS AND INFORMED CONSENT
All practitioners affiliated with this clinic provide services as independent business operators. Please contact your practitioner directly for any medical advice and queries.
Recovery: Remember that healing and recovery takes time and not everyone heals/recovers at the same rate. If at any time during your care, you do not feel that you are responding as well as expected we would ask that you discuss this with your practitioner.
Appointments: Your practitioner will outline a recommended action plan as the best plan for your injury. You will achieve the maximum results when you keep your recommended action plan to this schedule. Therefore, to receive the most out of your care and to save time we ask that you schedule your appointments in advance.
Cancellation Policy and Missed Appointments: Missed appointments will set you back in your recovery, so we ask that wherever possible you keep all your appointments. This Facility has a 24 hour cancellation policy for allied health services that applies to all appointments. Failure to provide 24 hours notice when changing or cancelling appointment times and missed appointments will result in being charged the full appointment fee. I understand that this cancellation fee may be waived in extreme or emergency circumstance by the practitioner, and solely at the Practitioner’s discretion. This fee is not covered by compensable bodies and must be paid by the patient.
Fees and Your Account: Fees for private patients are due at the time of service. HICAPS and EFTPOS facilities are available at the front desk for automatic claiming through your private health fund. Workcover and DVA patient accounts will be sent directly to the appropriate body. Should the claim be rejected in any way, the patient is responsible for payment of accounts.
Facility Use: By entering the Inertia Fitness facility, you agree to the Inertia Fitness’ Terms and Conditions (as found on your member profile in the app, and as located at reception for viewing and on demand).
Informed Consent:
When performed by a qualified allied health professional, treatment of the musculoskeletal system through the use of massage, stretching, and other soft tissue techniques is an effective and safe method for many conditions. There are, however, risks associated with any treatment that we are required to inform you of, although there has never been a case at this facility. Practitioners will discuss your condition and concerns and options for treatment with you so that you are appropriately informed and can make decisions on your treatment. You may choose to consent to, or refuse, any form of treatment for any reason including religious and personal reasons. Once you have given consent, you may withdraw consent at any time by notifying the practitioner.
Please read the following carefully and discuss any questions you may have with your treating practitioner. If you agree, please sign and return this form to them.
– I request and consent to the performance of exercise physiology, physiotherapy and associated allied health therapies by the practitioner/s at Inertia Fitness.
– I confirm that I will have the opportunity to discuss with the practitioner the nature and purpose of the exercises, therapies, and techniques. I understand that results are not guaranteed.
– I understand that with all medical treatments and assessments there are inherent risks and benefits. I do not expect the practitioner to be able to anticipate all of the risks and possible complications. I wish to rely on the practitioner exercising their judgement during the course of my treatment in the manner that is in my best interests, based on the facts known and information I have disclosed. I understand at times the practitioner may inform me, or request that I read and understand information related to a particular treatment and that the practitioner may request I sign a further consent form to ensure I fully understand the risks associated to my particular condition. I understand I may withdraw my consent at any time, even if I have previously signed a consent form.
– Consent from a custodial parent is required to treat a minor.
– Where a person is incapable of understanding the risks and benefits associated with treatment, consent may be provided by another person legally authorized to provide such consent. Evidence of legal authorization if required in such circumstances.
– I understand that during the examination and treatment, it may be necessary for your practitioner to make physical contact. All physical contact requires consent, and I understand my rights to withdraw consent at any time by notifying the practitioner who will immediately and safely cease contact. I understand that some assessments and treatments that are recommended may not be able to be completed without physical contact.
– I consent to photo and video recording where deemed appropriate by the therapist for biomechanical assessment.
– I have read the above, and confirm that I will have the opportunity to ask questions about its content. I intend this consent form to cover the entire course of treatment for my present condition, and for any other future condition/s.
– I understand that I can withdraw my consent at any time in writing.
– I understand that future appointments at this facility may require to be block booked to ensure optimal outcome for my condition, illness or injury.
– I consent to receiving appointment reminders, via email, SMS, phone, or app notifications, and other information from time to time regarding the services at Inertia Fitness, and that I can opt out of these notifications if requested.
– In understand that Inertia Fitness and all associated Practitioners comply with the Information Privacy Act 2009 (QLD) and that my personal information will be stored securely and only used for the purposes of providing health services.
